Standard Verification of Employment Response Form

After reviewing responses from groups such as the APA Government Affairs Task Force, the Office of Child Support Enforcement (OCSE) realized the burden on employers completing the varying Verification of Employment forms from each state.  In an effort to help the employer, they analyzed each state’s form and development a standard form which contains much of the information required by each state.

The OCSE contacted each state and many, though not all, states have accepted the form; this will reduce the number of forms the employer must decipher and complete.

The OCSE has also provided a listing of which states have or have not accepted this form.
